29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) New commercial construction located at 3225 Friendship Rd., Buford, GA 30519, Roof: On or about June 18, 2025, at a new commercial construction site located at 3225 Friendship Rd., Buford, GA 30519, employees performing decking operations were exposed to fall hazards at approximately 23 feet above the lower level. Employees climbed over the guardrails of a scissor lift to access the roof, resulting in a loss of fall protection during the transition from the lift to the roof surface.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a) New commercial construction located at 3225 Friendship Rd., Buford, GA 30519, Roof: On or about June 18, 2025, employees performing roof decking work were exposed to fall hazards of 23 feet to the lower ground level, in that employees were not provided with a fall protection system while engaged in a steel erection activity.
