29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). Location: Site On or about 6/17/2025, the employer failed to implement and maintain a safety and health program that ensured frequent and regular inspections of the jobsite by a designated competent person. At the time of inspection, the competent person did not identify or address ladder-related hazards present on site, nor did they identify instances of employees not wearing required personal protective equipment (PPE).
