29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) 333 New Road, Parsippany, NJ 07054 The employer did not establish an energy control program (LOTO program) for numerous machines and equipment operated at the facility, including but not limited to the Tilting type paste extruder - Model : JEC-PE-36-400, exposing employees to various hazards including, but not limited to, electrical and crush hazards. Violation occurs 06-10-2025

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) 333 New Road, Parsippany, NJ 07054 The employer allowed the employees to perform adjustments on a tilting type paste extruder - Model: JEC-PE-36-400 extrusion machine without first developing a specific procedure for isolating and locking out the machine. An employee sustained a laceration injury to the hand on the machine without isolating it before conducting the adjustment. Violation occurs on 06-10-2025

Parsippany, NJ 07054 The employer failed to install a guard on the rotating pulleys on the extrusion machine, exposing employees to nip point hazards. An employee sustained laceration injuries from the unguarded rotating pulleys. Violation occurs 06-10-2025
