Broad St. Philadelphia, PA: On or about June 17, 2025 - The employer did not inspect the scaffold and scaffold components for visible defects by a competent person before each shift. Employees worked on uninspected scaffolding while preparing to conduct carpentry activities, thereby exposing an employee to fall hazards of approximately 13 feet and struck-by hazards from vertical lifelines tied to the guardrail of the suspended scaffold.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) Jobsite located at 1805 N. Broad St. Philadelphia, PA: On or about June 17, 2025, the employer did not train an employee conducting carpentry activities on suspended and supported scaffolds to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards. Employees were using a supported scaffold, approximately 13 feet and 10 feet in height, with no fall protection to access a suspended scaffold and vertical lifelines for the suspended scaffold were tied to the guardrails of the scaffold, thereby exposing employees to fall and struck-by hazards.
