29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  (a) On or about June 17, 2025, the employer did not ensure that frequent and regular jobsite inspection were conducted to ensure employees were protected from hazards associated with their work. Hazards identified on the jobsite include but not limited to lack of training, incorrect setup of an extension ladder, and no fall protection equipment installed.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about June 17, 2025, the employer did not ensure that employees performing re-roofing on a single-story garage roof were protected from falling to the ground below.  This exposed employees to a fall hazard of approximately 8.5 feet onto roofing material and concrete.

(a) On or about June 17, 2025, the employer did not identify and correct employees securing fall protection ropes to the front of their harnesses. This exposed the employees to spinal injuries in the event of a fall.

(a) On or about June 17, 2025, the employer did not ensure the extension ladder used by employees to access and egress a residential roof was extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately 8.5 feet to the ground below.
