29 CFR 1926.651(k)(1):An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift. 87 Miller Road South Windsor, CT 06074: On or about June 18, 2025, no inspections of the excavation and adjacent areas were conducted by a competent person and no trench box was being utilized thereby exposing the employees working in the trench to possible cave-ins.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). 87 Miller Road South Windsor, CT 06074: On or about June 18, 2025, employees were tasked with working in a trench that was not protected from cave-ins. A trench box was available onsite but was not in use when employees were working in the trench.
