29 CFR 1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. On or about June 16, 2025, the employer did not ensure that the electrical outlet located near the desk area of the office in the truck repair shop had a faceplate to prevent accidental contact with live electrical components.

29 CFR 1910.37(b)(5):Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet). a. On or before June 16, 2025, the employer did not ensure that a doorway located in the office area of the truck repair shop that was not intended to be used as an emergency exit was clearly marked as such. The doorway was marked with both an illuminated exit sign and a paper "Not an Exit" sign.
