29 CFR 1926.502(h)(1)(ii): The safety monitor did not warn the employee when it appears that the employee is unaware of a fall hazard or is acting in an unsafe manner. a) Holicong Middle School, 2900 Holicong Road, Doylestown, PA: On or about June 21, 2025, while employees were working on the edge of a flat roof laying down rubber roofing material approximately 30 feet above the ground, an employee who was acting as the safety monitor did not warn another employee of a fall hazard as that employee was approaching the roof edge.

29 CFR 1926.502(h)(1)(v): The safety monitor was engaged in other responsibilities which took the monitor's attention from the monitoring function. a) Holicong Middle School, 2900 Holicong Road, Doylestown, PA: On or about June 21, 2025, employees were installing rubber roofing material on a flat roof approximately 30 feet above the ground and the safety monitor was engaged in other responsibilities, such as unrolling the rubber roofing, which took his attention away from the monitoring function.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and each employee was not trained in the procedures to be followed in order to minimize these hazards. a) Holicong Middle School, 2900 Holicong Road, Doylestown, PA: On or about June 21, 2025, employees were engaged in replacing rubber roofing material on a flat roof approximately 30 feet above ground level near the edge of the roof and employees were not trained to minimize fall hazards nor the employer's program to use safety monitors.
