29 CFR 1926.404(b)(1)(i):  Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  A.) 4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing a flexible electrical extension cord to power a relocatable power tap which provided power to electrical equipment including, but not limited to, battery chargers.  The flexible electrical extension cord, relocatable power tap, and electrical equipment were not being used in conjunction with a ground-fault circuit interrupter and/or an assured equipment grounding conductor program, on or about June 17, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(b)(1):  Conductors entering boxes, cabinets, or fittings were not protected from abrasion, openings through which conductors enter were not effectively closed, and unused openings in cabinets, boxes, and fittings were not effectively closed:  a.)  4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing an energized metallic outlet box to power equipment for work operations.  A yellow junior service flexible cord was wired into a metallic outlet box.  The inner insulated conductors of the junior service flexible cord were exposed to damage on the metal edge of the knockout opening and the opening was not effectively closed as the metal knockout bushing was nor secured into the knockout hole, on or about June 17, 2025.  b.)  4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing an energized metallic outlet box to power equipment for work operations.  A metal knockout on the side of the metallic outlet box was missing, exposing employees to shock hazards while handling the box, on or about June 17, 2025.  c.)  4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing an energized metallic outlet box to power equipment for work operations.  A yellow junior service flexible cord was wired into the rear of a metallic outlet box.  The inner insulated conductors of the junior service flexible cord were exposed to damage on the metal edge of the knockout opening and the opening was not effectively closed as the metal knockout opening was not equipped with a means to protect the inner insulated conductors and/or effectively close the knockout opening, on or about June 17, 2025.  d.)  4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing an energized metallic outlet box to power equipment for work operations.  An orange junior service flexible cord was wired into the side of a metallic outlet box.  The inner insulated conductors of the junior service flexible cord were exposed to damage on the metal edge of the knockout opening and the opening was not effectively closed as the metal knockout opening was not equipped with a means to protect the inner insulated conductors and/or effectively close the knockout opening, on or about June 17, 2025.  e.)  4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing an energized metallic outlet box to power equipment for work operations.  A orange junior service flexible cord was wired into a metallic outlet box. The metal outlet box was not was not installed and used in accordance with instructions included in the listing, labeling, or certification of the product in that metallic box was on the floor and was not flush mount to a surface, on or about June 17, 2025.  b) 4600 Jonestown Road, Harrisburg, PA, former Bon-Ton / new Fun City section of the Colonial Park Mall - Employees were utilizing a metallic outlet box to power equipment including, but not limited to, a portable jobsite light tripod.
