29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) At or near 6947 Ohio River Boulevard, Ben Avon, PA: On or about June 20, 2025 during roofing activities, the employer did not ensure the ladder used to get up/down from the porch roof was properly set up in that it only extended approximately 1 1/2 feet above the roof line.

29 CFR  1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer:    a) At or near 6947 Ohio River Boulevard, Ben Avon, PA: On or about June 20, 2025 during roofing activities, the employer did not ensure a system of hazard recognition and corrective actions was implemented at the site. Deficiencies included, but were not limited to, climbing over the motors of the shingle lifter to get on/off of the main house roof ladder, not using eye/face protection while operating power tools, not using fall protection and not having ladders properly extended above the roof line.
