29 CFR  1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about June 18, 2025, at a residential new build job site, located at 1263 Sandy Lake Rd., Ravenna OH 44266, the employer failed to conduct site inspections. These inspections aid in determining what potential hazards may exist and how to protect employees from such by implementing the use of various personal protective equipment, such as but not limited to the use of a protective eyewear when using nail guns and personal fall protection and/or systems.

29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about June 18, 2025, at a residential new build job site, located at 1263 Sandy Lake Rd., Ravenna OH 44266, the employer failed to ensure that employees, who were operating a pneumatic nail gun while performing roofing activities, were wearing protective eyewear.  Employees were exposed to flying debris and nails from the nail guns.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 18, 2025, at a residential new build job site, located at 1263 Sandy Lake Rd., Ravenna OH 44266, the employer failed to ensure employees, who were working on a residential roof of approximately 27 feet, were protected from a fall from height hazards in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.

Employees were exposed to a fall from height hazard of 27 feet.
