29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:   a) 10717 Georgetown Road, Laurel, DE 19956: On or about June 23, 2025, employees were exposed to amputation hazards when energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be used for the control of hazardous energy while accessing grain bins to remove grain and/or conduct maintenance.  NOTE: The procedure must contain information which authorized employees must know to safely control hazardous energy. Over generalization can result in a document which has little or no utility to the employee who must follow the procedure. However, while the procedure is required to be written in detail, this does not mean that a separate procedure must be written for each and every machine or piece of equipment. Similar machines and/or equipment (those using the same type and magnitude energy) that has the same or similar types of controls can be covered with a single procedure.

One of the employees pant legs was caught with the axle (bolts) creating a compound fracture to the employee.

29 CFR 1910.272(g)(1)(i): The permit for entering bins, silos, or tanks in the grain handling facility did not certify that the precautions contained in 29 CFR 1910.272(g) had been implemented prior to employees entering the bins, silos, or tanks:  a) 10717 Georgetown Road, Laurel, DE 19956: On or about June 23, 2025, employees were exposed to amputation as well as environmental hazards when they entered bins without confined space permits being completed showing required precautions have been implemented, all mechanical, electrical, hydraulic and/or pneumatic equipment was locked out and tagged out, the atmosphere inside the bin was tested for the presence of combustible gases, vapors, and/or toxic agents, and toxicity or oxygen deficiency eliminated.
