29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: D1 area by the Spex Victory 19- Injection Molding Machine - Employees performing daily operations were exposed to electrical shock hazards in that the Air Core On-Off switch faceplate was partially broken. Employees were permitted to use, walk, and work near and around the switch that did not have a fully covered plate to prevent contact with the live 120-volt inner conductors and components of the switch.
