29 CFR  1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity):   a) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a Fraco Act 8 Mast Climbing Work Plaform that lacked guard railing on the ends.  b) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a fabricated frame scaffold that lacked guard railing on the ends.  c) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height from a fabricated frame scaffold that lacked guard railing on the end frames while installing masonry units.   IN THE ALTERVINATIVE   29 CFR  1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a Fraco Act 8 Mast Climbing Work Plaform that lacked guard railing on the ends.  b) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height while installing masonry units from outriggers on a fabricated frame scaffold that lacked guard railing on the ends.  c) Jobsite located at or near 5800 City Ave. Philadelphia, PA- On or about June 24, 2025, employees were exposed to fall hazards of 60 feet in height from a fabricated frame scaffold that lacked guard railing on the end frames while installing masonry units.

29 CFR  1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:   a)Jobsite located at or near 5800 City Ave. Philadelphia, PA - On or about June 24, 2025, the employer did not ensure that employees working from the 10th level of a fabricated frame scaffold were protected from fall hazards of approximately 60 feet to a lower level by ensuring either a midrail or a top rail was installed with the cross brace as applicable.
