29 CFR 1910.28(b)(3)(i):The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) Roof: On or about June 23, 2025, employees were exposed to a fall hazard of approximately 17 feet, 9 inches to the lower level while installing or maintaining weather data equipment, in that the skylights present on the roof were not guarded, covered, or otherwise protected, thereby exposing employees to a fall hazard through the unprotected skylight openings.
