29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. A) Coastal Insulation - 171 Androvette Ave., Staten Island NY 10302 - The employer failed to protect workers from fall hazards, resulting in an employee falling approximately 9 feet from the attic to the lower level through an unguarded floor opening intended for a staircase that had not yet been constructed. No fall protection measures were in place at the time of the incident. Incident occurred on June 24, 2025
