29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  313 South Church Street, Moorestown, NJ: Employees exposed to head injury from impact, or from falling or flying objects while walking on the ground level while debris was being thrown from the roof level. Condition noted on or about 06/25/2025.  The Alfaros Home Business LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1798221, citation number 1, item number 1 and was affirmed as a final order on 05-27-2025, with respect to a workplace located at 652 Harvard Street, New Milford NJ.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 313 South Church Street, Moorestown, NJ: The employer did not ensure that employees were protected from falls.  Employees engaged in residential roof replacement were working on a roof roughly 20 feet to the ground surface and were not protected from falls by use of a personal fall arrest system or guardrail system. Condition noted on or about 06/25/2025.  The Alfaros Home Business LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1798221, citation number 1, item number 2 and was affirmed as a final order on 05-27-2025, with respect to a workplace located at 652 Harvard Street, New Milford NJ. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 313 South Church Street, Moorestown NJ: 3 Workers were exposed to fall hazards, up to and in excess of 20 feet to the ground surface below, when accessing to and egressing from a pitched roof using a portable ladder whose side rails did not extend at least 3 feet above the surface being accessed/egressed. Condition observed on or about 06/25/2025.  The Alfaros Home Business LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1798221, citation number 1, item number 3 and was affirmed as a final order on 05-27-2025, with respect to a workplace located at 652 Harvard Street, New Milford NJ.
