(a) 114 Apple View Drive, State College, PA: On or about June 25, 2025 an employee was in the basket of an aerial lift approximately 18 feet above grade and was not utilizing a fall protection system and was not tethered to the boom/basket. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

1926.1060(b) Retraining shall be provided for each employee as necessary so that the employee maintains the understanding and knowledge acquired through compliance with this section. (a) 114 Apple View Drive, State College, PA: On or about June 25, 2025 three employees of Eagle Wing Enterprises were observed working on a house roof in excess of 18 feet above grade. Roof access was utilizing a portable ladder that the siderails were only extended approximately 16 inches above the roof edge. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

1926.503(c)(3) Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. (a) 114 Apple View Drive, State College, PA: On or about June 25, 2025, three employees of Eagle Wing Enterprises were observed working on a house roof in excess of 18 feet above grade with no fall protection systems utilized. Eagle Wing Enterprises was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1670101, citation number 1, item number 1a and was affirmed as a final order on July 23, 2023, with respect to a workplace located at Corner of Pine Street & South Logan Boulevard, Altoona PA 16602. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
