29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  A) Salem Tube, Inc. on or about June 17, 2025 at the address of 485 Nickle Street, Greenville, PA: One Employee suffered a finger amputation from an unguarded pinch wheel moving the piping down Draw Bench #9 on the infeed side of the saw. The employee reached in to roll the pipe and caught their finger between the pipe and a roller, causing a pinch point resulting in an amputation.  B) Salem Tube, Inc. on or about June 17, 2025 at the address of 485 Nickle Street, Greenville, PA: No guarding is provided on the outfeed side of the chop saw at the pinch wheels. Employees are exposed to amputation hazards from tightening pinch wheels.   C) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to point of operation hazards on Draw Bench #9 on the infeed side of the machine. Moving from the East side of the building to the west towards the starting point of Draw Bench #9.  D) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to point of operation hazards on Draw Bench #9 on the infeed side of the machine. Moving from the East side of the building to the west towards the starting point of Draw Bench #9 at the grabbing mechanism.   E) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to in running nip points on Draw Bench #9 on the outfeed side of the machine.   F) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to moving parts on Draw Bench #9 on the outfeed side of the machine. A guard is provided to the right of the control panel but the guard still allows access for employees.  Employees can reach around the guard.  G) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to a Pulley on the return side of Draw Bench #9 on the outfeed side of the machine. A pulley is partially guarded but the outfeed side of the pulley is not protected from employees in that area.  H) Salem Tube, Inc. on or about June 25, 2025 at the address of 485 Nickle Street, Greenville, PA: Employees are exposed to nip points from the running conveyor belt and pulley not being guarded at the end of each rotating pulley and belt.
