29 CFR 1910.146(c)(3): When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit space and did not comply with paragraph (c)(2). Cave Springs Water Facilities: On June 24, 2025 and at times prior thereto, Great Basin National Park did not inform exposed employees that the confined space where the underground water pipes were located was permit-required by signs or other equally effective means as required by 1910.146(c)(2). This condition potentially exposed employees to permit-required confined space hazards.
