29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation and/or nip points: a. At the facility located at 627 North Lane Avenue, Jacksonville, Florida: On or about June 21, 2025, and at times prior, on Line 104 in the production area of the facility, the employer exposed employees to an amputation hazard, in that the Upper 1008D Winder Shaft did not have a guard properly installed to provide protection of nip points while employees were loading/unloading cores on the shafts. b. At the facility located at 627 North Lane Avenue, Jacksonville, Florida: On or about June 21, 2025, and at times prior, on Line 104 in the production area of the facility, the employer exposed employees to an amputation hazard, in that the Lower 1008D Winder Shaft did not have a guard properly installed to provide protection of nip points while employees were loading/unloading cores on the shafts. c. At the facility located at 627 North Lane Avenue, Jacksonville, Florida: On or about June 21, 2025, and at times prior, on Line 105 in the production area of the facility, the employer exposed employees to an amputation hazard, in that the Upper 1008D Winder Shaft did not have a guard properly installed to provide protection of nip points while employees were loading/unloading cores on the shafts. d.
