Site: 329 Speedwell Ave, Morris Plains, NJ 07950. 1). On or about June 25, 2025, DC Express Construction LLC employees engaged in the removal of old rubber membrane/tar from a pre-existing roof platform approximately 25 feet from the ground below were observed working at the edge of the unprotected side and were exposed to fall hazards.

29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: Site: 329 Speedwell Ave, Morris Plains, NJ 07950. 1). On or about June 25, 2025, DC Express Construction LLC failed to erect a warning line system 6 feet from the roofing edge, exposing employees engaged in removing and replacing rubber membrane/tar on a 25-foot-high building to the hazard of falling.

29 CFR 1926.502(h)(1)(v):The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: Site: 329 Speedwell Ave, Morris Plains, NJ 07950. 1). On or about June 25, 2025, DC Express Construction LLC failed to ensure it's employee who was assigned as the monitor for a warning line system during a roof replacement installation on a flat roof had no other responsibilities other than monitoring the system. This hazard exposed employees to possible fall hazard of approximately 25 feet from the ground below.
