29 CFR 1926.502(h)(1):The employer shall designate a competent person to monitor the safety of other employees and the employer shall ensure that the safety monitor complies with the following requirements: On or about June 27, 2025, during the installation of roof flashing material and removal of roof construction debris on a two story commercial building located at 16720 Chagrin Boulevard, Shaker Heights, Ohio, the employer failed to ensure the employee designated as a safety monitor was competent to the monitor the safety of employees working on a low-slope roof at a height of approximately 25 feet (North roof) and approximately 16 feet (South roof) above the ground. The employer failed to ensure the safety monitor was on the same walking/working surface of the employees being monitored [per 1926.502(h)(1)(iii)]. The employer failed to ensure the safety monitor had no other responsibilities which could take the monitor's attention from the monitoring function [per 1926.502(h)(1)(v)].
