29 CFR  1910.37(a)(3):  Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level:  On or about June 17, 2025, the employer failed to ensure that the emergency exit route in the rear of the facility next to the Magellan Box Maker Machine (serial #MB-1027), was unobstructed.  The Magellan Machine was set up within the exit route, and when in operation, the work table, lift table, product, power cord and employee operators were directly in the exit route, obstructing access to the emergency exit door.

29 CFR  1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   On or about June 17, 2025, the employer failed to ensure that one or more methods of guarding was in place on the rear belt and pulley system (timing belt) on the  # 20 Martin FFG press (serial #171).  Employees were exposed to struck by and/or caught in between hazards when reaching over the conveyor assembly to clear a jam.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about June 17, 2025, the employer failed to review the machine specific procedures for the control of hazardous energy at least annually to correct any inadequacies, ensure that the procedures and the requirements of the standard were being followed and that the procedures were effectively implemented within the facility, for machinery such as but not limited to the  # 20 Martin FFG press (serial #171) .  Employees were exposed to hazardous energies including electrical and pneumatic when clearing jams.  Vail Industries, Inc., DBA Massillon Container Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.147(c)(6)(i), which was contained in OSHA inspection number 1579075, citation number 1, item number 1 and was affirmed as a final order on November 20, 2022,  with respect to a workplace located at 49 Ohio Street SW, Navarre OH 44662.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a) On or about June 10, 2025, employees were exposed to machine hazards associated with moving parts and in-going nip points while  performing the task of clearing jams on the # 20 Martin FFG press (serial #171).   The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the task of clearing a jam [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(S)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from these hazards.  b) On or about July 30, 2025,employees were exposed to machine hazards associated with moving parts and in-going nip points while performing the task of cleaning the anilox print rollers on the # 20 Martin FFG press (serial #171). The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the task of cleaning the anilox print roller [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(S)(i)J, and verification of isolation [(d)(6)], were not implemented to protect employees from these hazards. 29 CFR  1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:  On or about June 17, 2025, the employer failed to ensure that one or more methods of machine guarding was in place on the rear of the #15 Slitter Machine (serial #15901) where the multiple blades are located at the point of operation.   Employees were exposed to the point of operation hazards at the Slitter blades and where the cardboard product discharges off the roller barrels.  Vail Industries, Inc., DBA Massillon Container Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.212(a)(1) , which was contained in OSHA inspection number 1579075, citation number 1, item number 5 and was affirmed as a final order on November 20, 2022,  with respect to a workplace located at 49 Ohio Street SW, Navarre OH 44662.
