29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) On June 23, 2025, the framed scaffolding that the employees were working from had base plates that were supported by unstable wooden blocks.  No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) On June 23, 2025, employees working from frame scaffolding were exposed to the hazard of falling approximately 15 feet to the ground below.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On June 23, 2025, employees applying stucco to the exterior of an existing two-story residential structure from the roof  were not protected from fall hazards while working at an elevation of approximately 12 feet above the ground.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
