(a)5555 Whittlesey Blvd SUITE 2550, on June 25, 2025 and at time prior: The employer exposed employees for falls of 11 ft from the flat awning roof to the concrete below, in that use of the persona fall arrest system was not enforce while employees were installing new roof membrane.
