29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Residential Roof: On June 25, 2025, an employee operated and assisted with a handheld grinder to cut and shape flashing around the chimney on a residential roof without wearing impact rated safety glasses with side shields, exposing the employee to eye injuries from flying debris. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). (a) Residential Roof: On June 25, 2025, employees were performing flashing work around the chimney on an approximately 8/12 pitched section of the residential roof at heights of up to 18 feet 6 inches above grade without the use of fall protection, exposing the workers to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
