29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s).  Jobsite: 3766 Jeffrey Ct. in Arnold, MO. On or about 6/25/2025, One Way Roofing LLC did not conduct an adequate inspection of the job site, materials, and equipment. Employees were exposed to the following hazard:  a) The roofing subcontractors, KNC General Construction were removing old roofing materials from a residential structure approximately 10 feet above ground on a 6/12 pitched roof. The crew was not utilizing proper personal fall arrest systems (PFAS).  b) The general contractor controlling the work was onsite monitoring the work during the time of the inspection.
