29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 6/26/2025, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 5365 Burgundy Ct in Imperial,  MO. The employees were conducting residential roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 6/26/2025, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing at the job site located at 1025 Vogel Estates Lane, in Arnold MO. The ladder the employees used to access the roof was extended approximately .5 feet beyond the upper landing, or edge of the roof, thus exposing employees to fall hazards.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about 5/26/2025, the employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record each employee who were engaged in roofing operations 6ft (1.8m) or more above lower levels. The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury while at the job site located at 5365 Burgundy Ct, in Imperial, MO. The employees were conducting roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.
