29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:  a.)  Genesis Roofing and Construction, LLC.;  552 W. 12th Street, Erie, PA - On or about 7/1/25, and at times prior thereto, employees were engaged in roofing activities on a flat roof.  There was an area on the southeast corner of the lower roof that was missing a section of the guardrail system.  Workers were exposed to fall hazard of 15 feet.  b.)  Genesis Roofing and Construction, LLC.; 552 W. 12th Street, Erie, PA - On or about 7/1/25, and at times prior thereto,  employees were engaged in roofing activities on a flat roof.  There was an area on the southwest corner of the lower roof that was missing a section of the guardrail system creating a potential fall hazard.   Workers were exposed to fall hazard of 15 feet.  c.)  Genesis Roofing and Construction, LLC.; 552 W. 12th Street, Erie, PA - On or about 7/1/25, and at times prior thereto, employees were engaged in roofing activities on a flat roof.  There was a warning line being used on the higher roof but employees were working outside of the warning line system.  Workers were exposed to a fall hazard of 7'6" to the lower roof and 22' from the front of the roof to the ground below.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR  1926.503(a)(2):The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a.)  Genesis Roofing and Construction, LLC; 552 W. 12th Street, Erie, PA - On or about 7/1/25, and at times prior thereto,  employees had not been trained on the requirements noted in the 29 CFR Subpart M.  Employees were conducting roof work operations and were exposed to falls up to 15 feet.  b.)  Genesis Roofing and Construction, LLC; 552 W. 12th Street, Erie, PA - On or about 7/1/25, and at times prior thereto,  employees had not been provided with training on the requirements of  the safety monitoring system prior to using the safety monitor system for the method of fall protection.  Employees were conduction roof work operations and were exposed to falls of 15 feet.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).
