29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees selected by, and under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  Location: Job site;  On or about 07/01/2025, employees were allowed to utilize Alum-A-Pole pump jack scaffolding which was not erected by experienced employees under the supervision of a competent person qualified in scaffolding erection. The deficiencies with the pump jack scaffolding units which were not correctly erected or whose deficiencies were not identified and corrected prior to usage by a competent person are noted in the subsequent violations in 1-1-b through 1-1-d.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation.  Location: Job site;  On or about 07/01/2025, the employer allowed employees to utilize 2 Alum-A-Pole pump jack scaffolding which did not bear on base plates, mud sills, or other adequate firm foundation.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  Location: Job site;  On or about 07/01/2025, employees were exposed to a live overhead electrical service drop which was entering the building from the utility pole which was in direct contact with the Alum-A-Pole pump jack scaffolding unit that was erected directly beneath the service drop and had been used to install new Dupont Tyvek HomeWrap to that side of the residence.

29 CFR 1926.451(g)(1)(vii): An employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location: Jobsite;  On or about 07/01/2025, employees were exposed to a fall of greater than 10-feet above the ground while performing siding work from a pump jack scaffold that was erected without a complete guardrail system or fall protection system to protect them.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location: Job site;  On or about 07/01/2025, employees were allowed to perform siding work from the lower roof of the residence approximately 7-feet above the next lower level without any form of fall protection, such as guardrails, safety nets, or alternative protective measures.
