29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: (a) On or about July 1, 2025, at a residential property located at 21755 E 65th Ave. in Aurora, Colorado, the employer failed to ensure that impromptu/jury-rigged devices were not used by employees to increase the working height levels when working from scaffold platforms. While in the process of prepping an exterior wall, an employee using a table for height elevation while working from tubular steel-framed scaffolding was exposed to a fall hazard from a height of approximately 40 feet to the ground below.

29 CFR 1926.451(b)(1): Platforms on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units: (a) On or about July 1, 2025, at a residential property located at 21755 E 65th Ave. in Aurora, Colorado, the employer did not ensure that the scaffold system(s) used by employees during the installation of weather-resistant barrier (WRB) was fully planked thus exposing the employees to falls, slipping on the unsecured planks, instability leading to collapse, and struck-by hazards for falling objects.

9 CFR 1926.451(h)(4)(ii): Toeboards were not securely fastened in place at the outermost edge of the platform and not have more than 1/4 inch (0.7 cm) clearance above the walking/working surface: On or about July 1, 2025, at a residential property located at 21755 E 65th Ave. in Aurora, Colorado, the employer failed to ensure that employees working at ground level and/or at the base of a five (5) tier scaffold system were protected from falling objects /struck-by hazards. During the installation of weather-resistant barrier, at various points on the scaffold, toe-boards to prevent tools, materials and/or equipment from falling to the lower level were not installed.
