29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about July 2, 2025, at the above addressed jobsite, an employee using a handheld circular saw did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about July 2, 2025, at the above addressed jobsite, employees performing residential roofing operations, approximately sixteen (16') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.  b. On or about July 2, 2025, at the above addressed jobsite, employees performing residential roofing operations, approximately nine (9') feet two (2) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Top Choice Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1779529, citation number 1, item number 1 and was affirmed as a final order on January 2, 2025, with respect to a workplace located at 15223 Teebrook Dr., Orland Park, IL 60462.  Top Choice Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1740092, citation number 1, item number 1 and was affirmed as a final order on May 6, 2024, with respect to a workplace located at 44 N. Vail Ave., Arlington Heights, IL 60005.  Top Choice Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1735478, citation number 1, item number 1 and was affirmed as a final order on July 15, 2024, with respect to a workplace located at 950 Patriot Ln., Hoffman Estates, IL 60192.
