This condition exposed the employees to fall hazards. (b)Greg Johnson Construction, Inc. at 760 West Highway 12, Webster, SD 57274: On or about and at times prior to July 2, 2025, the employer did not ensure adequate scaffolding systems were in place, in that planked scaffolding systems were not at least 18 inches for employee use on the east side of a commercial structure. This condition exposed the employees to fall hazards.
