29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Production Room, Line 1 Crystallizer The employer did not develop procedures to safely lock out crystallizers prior to servicing them. On or about June 30, 2025, an employee suffered a fingertip amputation while trying to clear a clog ( "Material bridge" ) from the crystallizer. Lacerta Group, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1694228, citation number 1, item number 1 and was affirmed as a final order on January 24, 2024, with respect to a workplace located at 360 Forbes Blvd. in Mansfield, MA.
