29 CFR  1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard:  On June 29, 2025, at a facility located at 4243 Gateway Drive in Sheboygan, Wisconsin; an employee was exposed to unprotected fall hazards of less than 4 feet into the circulation pump well of Furnace #15 containing molten aluminum at temperatures exceeding 1,200 degrees Fahrenheit when the employee was changing out a circulation pump.

29 CFR  1910.132(d)(1)(i):  The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment:  On June 29, 2025, at a facility located at 4243 Gateway Drive in Sheboygan, Wisconsin; the employer did not select and have affected employees use the types of PPE that would protect the affected employees from hazards identified by the employer's hazard assessment. The employer identified Furnace Operator employees were exposed to splashing molten metal hazards when conducting tasks such as, but not limited to, pouring/transferring, desludging, lance change out, and chlorine pump changes in the Alloy department. The employer did not select and have Furnace Operator employees wear leg protection for splashing molten metal.

29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:  At a facility located at 4243 Gateway Drive in Sheboygan, Wisconsin; the employer did not obtain prior written approval for additions made to forklifts, which affected their capacity:  a) On June 29, 2025, a Rig Ready Forklift Boom (Model QL-175) was affixed to the mast of a Hyster Fork Truck (Model S120FTS) and used to lift and lower circulation pumps into and out of Furnace #15's circulation pump well. The employer had not obtained written approval from Hyster to add the boom attachment to the forklift and did not update the capacity and operation plate to adjust the lifting capacity, exposing employees to tip over hazards.  b) On July 1, 2025, a Lift Master Telescoping Lift Boom (Model LM-1T) was affixed to the forks of a Hyster Fork Truck (Model S120FT-PRS) and used to lift and lower circulation pumps into and out of Furnace #10's circulation pump well. The employer had not obtained written approval from Hyster to add that boom attachment to the forklift's forks and did not update the capacity and operation plate to adjust the lifting capacity, exposing employees to tip over hazards.

29 CFR  1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  At a facility located at 4243 Gateway Drive in Sheboygan, Wisconsin; the employer did not request that forklifts be marked to identify front-end attachments, other than factory installed, to show the approximate weight of the truck and attachment combination at maximum elevation and with load laterally centered:  a) On June 29, 2025, a Rig Ready Forklift Boom (Model QL-175) was affixed to the mast of a Hyster Fork Truck (Model S120FTS) and used to lift and lower circulation pumps into and out of Furnace #15's circulation pump well. The forklift's name plate only contained combined weights for the truck and fork attachments.
