29 CFR  1926.304(h)(1):Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  a) 5817 West Bluff Drive, Keuka Park, NY 15578, at the jobsite, on or about 7/2/25:  Workers were exposed to cut hazards while using a DeWalt Circular saw without a guard.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) 5817 West Bluff Drive, Keuka Park, NY 15578, at the jobsite, on or about 7/2/25:  Workers were exposed to falls of 21 feet to ground, when accessing the pick of a pump-jack scaffold from a roof surface that was 36 inches from that roof surface.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a) 5817 West Bluff Drive, Keuka Park, NY 15578, at the jobsite, on or about 7/2/25:  Workers were exposed to falls of 21 feet to ground, when accessing the pick of a pump-jack scaffold and 24 feet to ground at the unprotected end of the pick without being trained to recognize those hazards.  Abatement certification must be provided for this item.
