29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   Location: Job site;  On or about 07/03/2025, an employee was witnessed working on a low-sloped roof without utilizing fall protection while transporting scrap and other roofing debris to the rollaway dumpster at the rear of the building.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached.  Location:  Job site;  On or about 07/03/2025, employees were allowed to use temporary roof anchors that were not installed according to manufacturer instructions to support 5,000 lbs per employee. The sole anchorage point was visibly lifting from the roof ridge, with unsecured holes and daylight visible beneath it.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  Location: Job site;  On or about 07/03/2025, employees were observed wearing fall protection harnesses incorrectly while working 11 feet or more above a lower level. The harness attachment points were positioned near the waist and lower back, rather than between the shoulder blades.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder.   Location: Job site;  On or about 07/03/2025, employees utilized two portable extension ladders at the front of the building to gain access to the upper levels of the structure that was being roofed and the ladders did not extend at least 3-feet above the area which they were being used to gain access to.
