29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. Naval Base San Diego, BLDG 71: On or about July 3, 2025, and at times prior thereto, employee(s) used a white hard hat which had approximately a 2-inch crack on the aft-end. The personal protective equipment used to protect the head was not maintained in a reliable condition. This condition potentially exposes employee(s) to reduced impact protection hazard(s).

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired. Naval Base San Diego, BLDG 71: On or about July 1, 2025, and at times prior thereto, employee(s) were working on a scaffold which was damaged with a bent cross bar located in the center west end of the structure. The cross bar was not replaced or removed from service. This condition potentially exposed employee(s) to fall hazard(s).
