1910.333(b)(2)(ii)(B): The circuits and equipment to be worked on shall be disconnected from all electric energy sources. Control circuit devices, such as push buttons, selector switches, and interlocks, may not be used as the sole means for deenergizing circuits or equipment. Interlocks for electric equipment may not be used as a substitute for lockout and tagging procedures. (a) 5020 Ivy St, Commerce City CO: On or about July 3, 2025, the employer did not ensure that equipment, including but not limited to motor control centers (MCCs), was de-energized before work began. The company's 480V Allen-Bradley Centerline MCC were not properly locked before maintenance mechanic began removing fuse. This condition exposed employees to hazards from arch flash and arc blast.

29 CFR 1910.269(l)(8)(v):The employer shall ensure that each employee exposed to hazards from electric arcs wears protective clothing and other protective equipment with an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeds 2.0 cal/cm2. This protective equipment shall cover the employee's entire body. 5020 Ivy St, Commerce City CO: On or about July 3, 2025, the employer did not ensure that each employee conducting maintenance on the 480V Allen-Bradley Centerline MCC wore protective electrical arc flash clothing.
