29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a) Kitchen - Dishwashing Station: On or about July 7, 2025, the employer exposed kitchen staff to slip hazards in that the floor in the dishwashing area was continuously wet during normal working hours, creating a risk of slips and falls while employees were washing dishes or passing through the area.  b) Dining Room - Door from Kitchen: On or about July 7, 2025, the employer exposed employees to slip hazards while entering or exiting the kitchen in that the anti-slip mat in front of the kitchen door was routinely removed because bus cart wheels and casters became caught on it, eliminating slip protection in that area.  c) Dining Room - Door From Kitchen: On or about July 7, 2025, the employer exposed employees to trip hazards while entering or exiting the kitchen in that a floorboard was curled from water damage, creating an approximately one-half inch raised edge.
