29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  Location: a) Supported scaffold in the jobsite:  On or about January 10, 2025, an employee was tied off to a supported scaffold frame that was attached with a hoist. During hoisting operations, the scaffold frame broke and fell, which pulled down the employee. The employee fell approximately 50 feet from the ninth floor level down to the fifth floor setback. The legs of the broken scaffold frame were found to be rusted with one side of the leg missing a pin. The competent person did not inspect the scaffold for defects prior to disassembling operation.

Location: a)	Supported scaffold in the jobsite:  On or about January 10, 2025, an employee was tied off to a supported scaffold frame that was attached with a hoist. During hoisting operations, the scaffold frame broke and fell, which pulled down the employee. The employee fell approximately 50 feet from the ninth floor level down to the fifth floor setback. The legs of the broken scaffold frame were found to be rusted with one side of the leg missing a pin. The scaffold frame was used as an anchorage as part of the personal fall arrest system, but the employer did not inspect it prior to the employee's use.

During hoisting operations, the scaffold frame broke and fell, which pulled down the employee. The employee fell approximately 50 feet from the ninth floor level down to the fifth floor setback. The legs of the broken scaffold frame were found to be rusted with one side of the leg missing a pin. The scaffold frame was not capable of supporting at least 5,000 pounds for the employee attached.
