29 CFR  1910.303(g)(1)(i)(A):  For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  Trailer Yard:  On, or about, July 7, 2025, the working space for the Eaton 480/277 volt panelboard labeled, "Parking Lot Sub Panel 1" was blocked by a concrete barrier and propane cylinder storage cage.

Adequate insulating protective equipment was not used.

29 CFR  1910.334(a)(2)(i):  Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Trailer Yard:  On, or about, July 7, 2025, the employer did not implement a program to visually inspect equipment related to the land power lines for the trailer freezers.  The cords and connectors were subject to damage from pedestrian and vehicular traffic.
