29 CFR 1926.20(b)(4): The employer failed to permit only those employees qualified by training or experience to operate equipment and machinery: a) Work site - On or about July 3, 2025 and at times prior thereto, an employee was unloading a 40 foot long irrigation pipe from a front loader tractor, when the pipe hit the control arm causing the loader bucket to lower onto an employees back side of the legs. The employee did not receive adequate training prior to operating the front loader tractor.

The employees were not instructed in the recognition and avoidance of unsafe conditions when loading and unloading 40 foot irrigation pipe from the tractor.
