29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).   (a) Covenant House, Charleston, WV Jobsite: On or about July 8, 2025, employees working from a pump jack scaffold approximately 22 feet above grade were exposed to fall hazards because the scaffold platforms were not fully planked or decked between the uprights/poles and pump jack brackets, leaving gaps of approximately 6 inches to 12 inches on either side of platforms that were approximately 12 inches to 18 inches wide, exposing employees to falls to a lower level.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

(a) Covenant House, Charleston, WV Jobsite: On or about July 8, 2025, employees working from a pump jack scaffold approximately 22 feet above grade were exposed to fall hazards because scaffold platforms greater than 10 feet in length extended approximately 9 feet to 12 feet beyond their supports on either side of the pump jack structural poles and were not designed or installed to safely support employees without tipping.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
