OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards during die key removal on forging hammers:  On or about July 7, 2025, the employer assigned employees to remove die keys from a forging hammer through a ramming operation and exposed them to struck-by hazards from the unsecured drift ejecting. An overhead crane with a ram attachment was handled by employees to repeatedly strike the drift and force the die key out. The hazardous conditions of this operation are likely to cause death or serious physical injury.
