29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. a) 160 Delaware Avenue, Buffalo, NY - On or about 7/3/25, an extension ladder, being used to access the roof, did not have the ladder side rails extended at least 3 feet above the upper landing surface being accessed exposing employees to a fall of approximately 20" to the concrete ground below. NO ABATEMENT CERTIFICATION REQUIRED

The employee did not grasp the ladder with at least one hand at all times. NO ABATEMENT CERTIFICATION REQUIRED
