OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  a) Production floor (Deburring area) - On or about, July 8, 2025, and time prior, the employer exposed employees to struck-by and/or crush-by hazards in that the four steel pillars/beams  that served/support the upper level product moves were not anchored to the floor. The pillars  were not  secured/anchored by means that would prevent them from being moved.

29 CFR  1910.146(c)(4):When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:  (a) On or about July 8, 2025,  and times prior, the employer failed to implement a written confined permit space entry program that complied with 29 CFR 1910.146 . The employer exposed employees to potential atmospheric and mechanical hazards in that the employer did not develop a site specific written Permit Required Confined Space program that: Established and implemented the means, procedures and practices to eliminate or control hazards necessary for safe permit space entry operations, Identified employee job duties, implemented appropriate procedures for summoning rescue and emergency services, and preventing unauthorized personnel from attempting rescue, and  Implemented a system for the preparation, issue, use and cancellation of entry permits.

29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in a downward or outward direction within 2 inches (5 cm) of the top edge, at any point along the top rail.  a) Production floor (Deburring area) - On or about, July 8, 2025, and time prior, the employer exposed employees to fall from heights greater than 4ft in that  the guard rails attached to the 2nd level walking working platform were not incomplete (missing sections that were replaced with fiber ropes) and not capable of withstanding at least 200lbs of force.

The employer exposed employees to potential atmospheric and mechanical hazards in that the employer did not develop site specific written Permit Required Confined Space training.
