29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) Line 4 - On or about July 22, 2025, employees working on the Line 4 film under winder platform were approximately 10 feet above the ground with unprotected edges exposing employees to fall hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.146(d)(3)(iii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including isolating the permit space:   (a) Line 4 : On March 4, 2025,  the employer did not develop and implement the means, procedures, and practices necessary to isolate gas valves on Line 4 prior to oven entries by employees for strip repairs.  (b) Line 4 : On March 22, 2025,  the employer did not develop and implement the means, procedures, and practices necessary to isolate gas valves on Line 4 prior to oven entries by employees for strip repairs.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.146(k)(1)(i):  When designating rescue and emergency services pursuant to 29 CFR 1910.146(d)(9), the employer did not evaluate the prospective rescuer's ability to respond to a rescue summons in a timely manner, considering the hazard(s) identified:  (a) Line 4 - On March 4, 2025, the employer did not evaluated the Elk Grove Fire Department's ability to respond to a rescue summons in a timely manner for oven entries prior to entry into those spaces.  (b) Line 4 - On March 22, 2025, the employer did not evaluate the Elk Grove Fire Department's ability to respond to a rescue summons in a timely manner for oven entries prior to entry into those spaces.
