29 CFR  1910.23(b)(13): The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder.   a) On or about July 3, 2025, at 705 Chickapee Trail. Maitland, FL: Employee was exposed to fall hazards, in that three points of contact were not used on a 12-foot fiberglass A-frame ladder while trimming tree branches at a height of 22 feet, while using a STIHL MS 460 Magnum chainsaw, power pruner.

29 CFR 1910.23(c)(7): The employer did not ensure that ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic are properly secured or guarded.  a) On or about July 3, 2025, at 705 Chickapee Trail in Maitland, FL, an employee was exposed to fall hazards while using a portable ladder to cut a tree branch approximately 22 feet high and 5-6 inches in diameter. The work was carried out without barriers or safeguards to prevent obstruction of traffic, increasing the risk of accidents.

29 CFR  1910.135(a)(1):The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  a) On or about July 3, 2025, at 705 Chickapee Trail. Maitland, FL:  employees were exposed to struck-by hazards, in that, the employer did not require the use of head protection while working near or under a tree limb being cut at a height of 22 feet with a diameter of 5-6 inches.

The employer notified OSHA on July 7, 2025 of the hospitalization.
